Medical ID Tag

Hemophilia of Indiana is in partnership with the American Medical ID to provide our patients with the most up to date Medical ID Tags. Most patients receive information regarding our MedicAlert Program when they visit the IHTC for the first time or when they attend their comprehensive clinic appointment. However, patients are more than welcome to contact the office directly to receive more information about the program. In order to obtain an American Medical ID membership and ID, members must fill out the designated American Medical ID application, if the member is a minor, their guardian must fill it out. Hemophilia gladly covers the initial membership cost and the first Medical ID Tag; however, after this the patient is then responsible for renewing the membership and replacing any damaged, lost, or out of date IDs.
