Hemophilia of Indiana is in partnership with the MedicAlert Foundation to provide our patients with the most up to date MedicAlert IDs. Most patients receive information regarding our MedicAlert Program when they visit the IHTC for the first time or when they attend their comprehensive clinic appointment. However, patients are more than welcome to contact the office directly to receive more information about the program. In order to obtain a MedicAlert membership and ID, members must fill out the designated MedicAlert application, if the member is a minor, their guardian must fill it out. Hemophilia gladly covers the initial membership cost and the first MedicAlert ID; however, after this the patient is then responsible for renewing the membership and replacing any damaged, lost, or out of date IDs. Contact Angel Couch at 317-570-0039 for more information.
